This are the forms clients will use for anything related to Insurance Reimbursements.
Bring everything in filled out by you and your Doctor. Client pays us for service they received. We then fill out all the paperwork on our end and return it to the client (No more then a two week period from the time of their visit). Then you send it to your Insurance provider for reimbursements.
*No discounts can be used when billing Insurance Companies.
